Withdrawn members may be reinstated into Phi Delta Kappa and any chapter of choice by completing this form The reinstatement fee is $10.00 plus payment of international and chapter dues for the current year.

Please note that withdrawn members are those who have voluntarily requested withdrawal or have been automatically withdrawn for nonpayment of dues for two consecutive years. Members not in good standing (nonpayment for 1-2 years) do not pay the reinstatement fee to regain good-standing status.
